A Rikers Island inmate died early Tuesday morning in an incident Department of Correction officials say involved “procedural violations.”

Felix Taveras, 40, reported a “medical condition” just before midnight on Monday, DOC officials said in a statement.

Taveras was then brought to one of the jail’s medical clinics and was later transported to Elmhurst Hospital where he was pronounced dead at 1:21 a.m., authorities said.

“Based on preliminary departmental review, procedural violations were discovered and suspensions will be issued,” DOC said in a statement.

It marks the third death of a person in custody at Rikers Island this year, according to DOC.

Sources say a fourth Rikers inmate died at Elmhurst Hospital earlier this year after being released following a medical incident. The individual was not in Department of Correction custody at the time of his death.

Last year, 19 incarcerated people died at Rikers Island jails, a historic high in New York City.

Taveras’ cause of death is under investigation by the city’s Office of the Chief Medical Examiner, officials said.

As is in the case of all deaths of people in city custody, the state Attorney General and the city’s Department of Investigation will launch inquiries into Taveras’ death.
