The New York City Housing Authority (NYCHA) announced administrative changes Friday after fatal elevator accidents in some of its buildings in 2015.

NYCHA's office said it has fired the head of its elevator division and is appointing a new chief. It is also disciplining five employees.

84-year-old Olegario Pabon died on Christmas Eve, after he was injured while riding a faulty elevator at the Boston Road Houses in the Bronx.

Officials said NYCHA was warned about the malfunctioning elevator an hour-and-a-half before the accident, but did not take any action.

A city investigation found various safety violations in dozens of NYCHA elevators, including broken brake monitors that are supposed to be fail-safe devices.

NYCHA said its staffing changes are part of an effort to improve elevator inspections and reduce emergency response times.
