The Medical Examiner's office says a 6-year old boy who was beaten to death last month in Harlem died of fatal child abuse syndrome. 

Zymere Perkins had evidence of acute and chronic abuse and neglect that led to his death, according to the Medical Examiner's office, which has ruled his death a homicide. 

Perkins died after his mother's boyfriend, Rysheim Smith, allegedly beat him with a broomstick. 

Smith and the child's mother, Geraldine Perkins, are charged with endangering the welfare of a child. 

The Administration for Children's Services came under fire for not following up on five warnings the child was being abused. 

Four managers were suspended without pay, and five workers involved in the case were placed on modified duty. 

The agency says it is still conducting a review of the case.
