It never should have happened.  That's the finding of an investigation into a building collapse last year in Travis that left a worker dead.

Formica Construction will have to pay $121,000 after the Occupational Safety and Health Administration found a disregard for federal safety protocols.  

Crews from Formica were tearing down the old Dana Ford building in November when the the 2nd floor came tumbling down on 43 year-old Delfino Jesus Velazquez Mendizabal.  He died of his injuries.  

An OSHA spokesperson said if Formica chose to plan and carry out the demolition correctly, the collapse would not have happened.  

The company has refused to comment on the incident.
