Health officials say more than 750,000 New Yorkers have a high risk of suffering a heart attack or stroke.
A health department report said the risk stems from dangerously high blood pressure.
The survey finds 26 percent of New Yorkers have high blood pressure.
The numbers are similar among men and women but differ sharply among racial and ethnic groups.
The study shows one-third of blacks suffer from high blood pressure compared to just 21 percent of whites and 26 percent of Hispanics.
Officials say the best way to counteract high blood pressure is to exercise, eat fruits and vegetables, limit salt intake and avoid smoking.
