Sources tell NY1 that multiple city agencies are under investigation for how they handled the case a of 4-year-old Bronx boy who died last week.

Police are still awaiting a medical examiner's report on Juan Sanchez, who died Sunday after being rushed to the hospital.

Sources say the boy may have been playing under a sink where chemicals were kept, and the ME took a sample of rat poisoning from that area.

Now, NY1 is told by sources that the Department of Investigation is looking into city practices surrounding the case, and the Administration for Children's Services is one of the agencies being investigated.

The deputy mayor of Health and Human Services also says she is ordering a full review of all Department of Homeless Services cases involving children in shelters.
