Police are investigating after a 2-year-old boy was found unconscious in a Queens home and later pronounced dead.

Police were called to the Naizer's Reid Jamaica home Tuesday and found him unconscious.

He was pronounced dead at the hospital.

The boy's mother was at the home at the time, but not the boy's father, according to sources.

Sources say Naizer was in ICU last month for respiratory issues he had since being born premature.

He had older siblings who helped look after him.

Neighbor: They take care of him, that make sure they feed him, make sure he's sleep, make sure he takes his medication and everything.
Q: And when you say take his medication?
Neighbor: He's kind of sickly, you know. He's in and out of the hospital.

The medical examiner will determine the cause of death.

Sources say police are questioning the boy's mother.
